Today, this makes me happy.

as a way for students to process
what got them into a time out

The behavior one of my students exhibited which caused him to have a time out and have to use this handy dandy reflection guide did not make me happy, but I am SUPER pleased that it worked.  He hand-wrote his answers (copied from the options on this page) on a separate sheet, and after class, we discussed his responses and how he can improve his behavior next time.  It was a very positive conversation, and I really think this new classroom management tool might be really successful.
